What information does Dan Zohar, P.A. collect?

When you visit the Web Site, you have the option to provide us with two types of information: (i) Personal Information you knowingly choose to disclose to us through the “Contact Us” webpage, or (ii) chat functionality and website use information collected by us as you interact with our Web Site (hereinafter “Anonymous Information”).

You can submit to Dan Zohar, P.A. certain information, including but not limited to, your name, address, email address, telephone and cellular telephone numbers, description of the accident or incident, injuries you sustained in the accident or incident, date of birth, and date of the accident or incident (collectively “Personal Information”). Google Analytics

We use a tool called Google Analytics to analyze trends, administer the site, track user movement, and to gather broad demographic information, along with 3rd-party audience data (such as age, gender, and interests) for aggregate use for reporting and donation purposes. The analytics tools we use collect only the IP address assigned to you on the date you visit this site, rather than your name or other identifying information. Although the analytics tools plant a persistent cookie on your web browser to identify you as a unique user the next time you visit this site, the cookie cannot be used by anyone but Google. Google’s ability to use and share information collected by Google Analytics about your visits to this site is restricted by the Google Analytics Terms of Use and the Google Privacy Policy.
